C-Arm for Vascular Surgery: Key Specifications That Define Performance

Vascular surgery has a complex relationship with C-arm technology. For decades, peripheral vascular and endovascular procedures were performed exclusively in fixed angiography suites — because fixed systems offered the image quality, detector size, and advanced imaging modes that these procedures required. The progressive improvement of mobile C-arm technology has changed that calculation, making a growing range of vascular procedures viable in the surgical suite with the right mobile system. Here is what the clinical requirements actually look like.
Detector Size: Why It Matters More in Vascular Work
In orthopedic fluoroscopy, the anatomy of interest — a hip joint, a pedicle screw, a wrist fracture — typically fits comfortably within a 21 cm detector field. In vascular fluoroscopy, the anatomy being tracked is often a catheter traveling through a vessel system that spans a large anatomical region: from the femoral puncture site through the iliac system into the aorta, or from the aortic arch through the visceral vessels. Procedures that require multiple table moves and C-arm repositions to cover the treatment zone introduce workflow complexity, increase procedure time, and require additional contrast injections for each repositioned acquisition.
A 30 x 30 cm detector field — available on systems like the Fujiflim Persona C-HR and the OEC Elite CFD — allows significantly more of the vascular anatomy to be captured in a single frame. For lower extremity runoff studies, iliac interventions, and aortoiliac disease evaluation, the difference between a 21 cm and a 30 cm detector often determines whether a single-position acquisition covers the clinical question or whether repeated repositioning is required.
Digital Subtraction Angiography (DSA)
DSA is the imaging mode that separates a vascular-capable C-arm from a standard surgical C-arm. In DSA, the system acquires a baseline image (the mask) before contrast injection, then subtracts the mask from each subsequent frame, leaving only the contrast-filled vessels against a clean background. The resulting images show vessel anatomy with dramatically improved conspicuity compared to fluoroscopy through bone and soft tissue — particularly important for small vessels, subtle stenoses, and distal runoff assessment.
Without DSA capability, contrast injections are visualized under standard fluoroscopy — the vessels are visible but compete with the overlying bone and soft tissue for visual attention. For diagnostic vascular work and for confirming treatment success after intervention, DSA-quality imaging is the clinical standard. Ensuring a mobile C-arm has DSA capability is a prerequisite for facilities intending to perform vascular procedures.
Roadmap and Overlay Functions
Roadmap mode overlays a fluoroscopic vessel map from a prior contrast injection onto the live fluoroscopy display, creating a real-time navigation guide for catheter and wire advancement. Without roadmap, the operator must periodically re-inject contrast to confirm catheter position relative to vessel anatomy — adding contrast volume, dose, and procedure time. With roadmap active, the live fluoro shows both the current catheter position and the vessel map simultaneously, allowing navigation through complex anatomy with fewer contrast injections.
Some systems provide overlay or fusion functions that allow the roadmap to be updated dynamically as the table moves, maintaining alignment between the vessel map and the current anatomy even after minor position changes. This is particularly valuable in tortuous vessel anatomy where catheter movement changes the relationship between the device and the surrounding structures.
Generator Power and Frame Rate
Vascular fluoroscopy often involves imaging through dense anatomical regions — the pelvis, the aortoiliac bifurcation — in patients whose body habitus may require higher output to achieve adequate image quality. A generator rated at 4 kW or above provides the output needed for these cases without heat throttling that degrades image quality at critical procedural moments. High frame rate DSA acquisition — 3 to 6 frames per second — is typically adequate for peripheral vascular work; cardiac applications may require higher rates, which is where fixed suites retain an advantage over mobile systems.
Bottom Line: For facilities evaluating whether to perform vascular procedures with a mobile C-arm, the key capability checklist is: 30 cm or larger flat panel detector, DSA mode, Roadmap functionality, generator power adequate for dense anatomy, and a generator and tube rated for the continuous fluoroscopy demands of vascular cases. Systems meeting these criteria bring credible vascular imaging capability to the surgical suite environment.
