The X-Ray Tube: The Single Biggest Line Item
The X-ray tube is the most expensive wear component in any CT scanner, and unlike most equipment failures, tube replacement is not a matter of if but when. Tubes have a finite life measured in heat units generated over thousands of scans, and high-volume facilities burn through that life faster than low-volume ones. Replacement tube costs vary significantly by scanner model and tube design, but facilities should budget for this as a near-certain expense within the system's operating life, not an unlikely contingency. Ask any vendor directly for expected tube life under your anticipated scan volume and the current replacement cost for that specific tube.
Service Contracts: What They Cover and What They Don't
CT service contracts typically come in tiers, from basic parts-and-labor coverage to comprehensive agreements that include preventive maintenance, software updates, and sometimes tube coverage. A contract that excludes tube replacement looks less expensive on paper but can leave a facility exposed to a very large unplanned cost. Read the fine print on exactly what triggers a covered repair versus a billable one, what the guaranteed response time is, and whether loaner equipment is provided during extended repairs. The cheapest service contract on the market is rarely the cheapest option once an uncovered failure occurs.
Detector and Generator Maintenance
Beyond the tube, the detector array and generator represent significant components with their own maintenance and calibration needs. Detectors generally have a longer functional life than tubes but require periodic calibration to maintain image quality and dose accuracy. Generators, particularly in older or heavily used systems, can develop faults that are expensive to diagnose and repair. Facilities should ask vendors about the typical maintenance schedule and cost history for these components specifically, not just the headline tube cost.
Software and Cybersecurity Updates
Modern CT scanners run on networked computing platforms that require ongoing software updates, both for new clinical features and for cybersecurity patching. Facilities connecting CT scanners to hospital networks and PACS systems need to budget for the IT and vendor support required to keep these systems current and secure, an ongoing cost category that older equipment purchasing models sometimes overlooked entirely.
Uptime Guarantees and What They're Worth
Many manufacturers, such as FujiFilm, now offer uptime guarantees, often citing figures around 99 percent, backed by remote diagnostic monitoring that can identify developing issues before they cause a failure. These guarantees have real value, but facilities should understand how uptime is calculated (scheduled maintenance windows are typically excluded) and what remedy applies if the guarantee is not met. A strong remote monitoring and diagnostic program, which several manufacturers now build into their support offerings, genuinely reduces unplanned downtime by catching problems early rather than waiting for a full failure.
Building a Realistic Maintenance Budget
A reasonable approach is to request, from any vendor under serious consideration, a total cost of ownership projection covering a five to seven year period, including expected service contract costs, a tube replacement estimate based on your anticipated volume, and any known history of major component costs for that specific scanner model. Comparing this total figure across vendors, rather than comparing purchase price alone, gives a far more accurate picture of what a CT scanner actually costs to own.
Bottom Line: CT scanner, maintenance costs, dominated by eventual tube replacement, service contract tiers, and ongoing software and calibration needs, can rival or exceed the purchase price over a system's operating life. Budget for these costs explicitly during the purchasing decision rather than treating the sticker price as the full financial picture.
